pt Health Stroke Rehabilitation
Comprehensive Stroke Care For Residents
Stroke is believed to be one of the leading causes of transfer of elderly individuals to long term care facilities (Statistics Canada, 2010). Of every 100 people who have a stroke, 15 die, 10 recover completely, 25 recover with a minor impairment or disability, 40 are left with moderate to severe impairment and 10 are so severely disabled they require long term care (Heart and Stroke Foundation, 2011). There are over 50,000 strokes in Canada each year and more than 10 % of patients who have experienced a stroke require long term care (Heart and Stroke Foundation, 2011).
A study by Stolee, Hillier, Webster and O’Callagha (2006) found the following limitations for stroke care in long term care facilities in Canada:
- Inadequate physiotherapy and occupational therapy services
- Poor transition from acute care to long term care
- Lack of coordinated and consistent care
- Limited input from residents and family members regarding rehabilitation goals
