Measurable, Goal Oriented Physiotherapy Care
For over 20 years pt Health has delivered OHIP funded physiotherapy care to Long Term Care and Retirement Homes across Ontario. pt Health physiotherapy is a one-to-one rehabilitation program designed specifically for older adults. A detailed hands-on assessment is performed by a registered Physiotherapist. Each assessment includes both physical and functional examinations and analysis, balance, gait and falls risk categorization. A measurable, goal oriented Physiotherapy care plan is recorded with each assessment
Eldercare Physiotherapy: The pt Health Approach
1. Assessment of Function
- History on the resident is collected which includes baselines, recent functional status and family/caregiver input
- Observations during ambulation and other activities of daily living are made and carefully noted
- Resident sensory capacity is determined including vision, hearing and cognition
- Standardized functional assessment instruments are used to accurately determine functional abilities of the resident
- Risk factors for causing functional decline are determined
- Determination of realistic functional capacity with physiotherapy intervention are made
2. Care Strategies
- Information about comfort, aides, devices etc are observed and gathered
- A physiotherapist designed exercise program is created. This program is tailored to each residents individual needs and incorporates many modalities to maximize function
- Pain management strategies are incorporated
- Recommendations of additional health care intervention if required
- Caregiver, resident or family education about condition is provided
- Documentation of intervention strategies and resident responses
3. Outcomes Identified
- Residents level of function
- Residents ADL
- Residents IADL
- Residents ambulation
- Residents fall risk
- Use of physical restraints
- Prevalence of residents who improve functional status
- Utilization of rehabilitation services – Compliancy
- Discharge and readmission rates

pt Health Physiotherapy services can provide treatments for each of those changes at no out-of-pocket costs to the resident or your home.

Who Pays?
In long term care, Physiotherapy is funded by OHIP. Each resident receives 100 visits per year with a possible extension of 50 extra visits if medically necessary. This equates to only 2-3 times a week that the physiotherapy team is able to see a resident for treatment. There is an option for family to pay for extra visits. Learn more about funding options.
