Eldercare Occupational Therapy Program

Maximizing The Functional Independence Of Your Residents

Occupational therapy is the art and science of enabling engagement in everyday living, through occupation; of enabling people to perform the occupations that foster health and well-being; and of enabling a just and inclusive society so that all people may participate to their potential in the daily occupations of life (Canadian Association of Occupational Therapists, 2007, p. 372).

pt Health’s Approach To Resident Occupational Therapy

Through the implementation of the pt Health Occupational Therapy Program, effective strategies and technique are developed to maximize the functional independence of your residents to enhance their quality of life while considering each resident’s unique needs within the facility environment.

From the initial referral to discharge, the resident is evaluated periodically to monitor changes so the treatment plan can be modified to best meet each resident’s unique needs.

Program Framework

The pt Health Occupational Therapy Program is developed based on two theoretical frameworks:

1. the Canadian Model of Occupational Performance and Engagement (CMOP-E)

2. the Canadian Practice Process Framework (CPPF)

Utilizing principles embedded in these two frameworks, our occupational therapists focuses on the health, safety and well-being of each resident and support evidence based care planning.

Occupational Therapy Service Types

The pt Health Occupational Therapy program contributes to the quality of care in long term care facility through the following services:

  • Cognitive training
  • Dementia care
  • Behavioural management
  • Falls prevention
  • Minimizing restraints
  • Staff in-service training
  • Restorative care
  • Activities of daily living retraining
  • Seating and mobility
  • Contracture management

Consistent with all the established programs at pt Health, the pt Health Occupational Therapy Program is delivered in a context of continuous quality improvement and follows an organized care pathway.
